Hy-Vee Health Exemplar Care is a membership-based healthcare organization headquartered in Des Moines, Iowa, founded by Dr. Jon Van Der Veer, DO. After beginning his adult medicine practice in a conventional way, Dr. Van Der Veer quickly identified that he wasn’t fulfilled with the “assembly line” approach to care and had a vision to do things differently.
Inspired by the direct primary care model, he found a way to treat people not symptoms. “Our model of care is focused on price transparency, affordability, access, and convenience,” emphasizes Dr. Van Der Veer. In January 2024, Exemplar Care announced its partnership with Hy-Vee, changing its name to Hy-Vee Health Exemplar Care. This partnership will extend this innovative healthcare model to even more individuals, ensuring excellent care remains accessible and affordable for all.
“Our priority is putting our members first,” says Dr. Jon. “We go one step further than most direct primary care by offering 24/7 urgent care and making sure that all of our on-site services are included in membership.
