Community recognition marks the organization’s second Gold award and fourth consecutive year of recognition
Hy-Vee Health Exemplar Care has been named the Gold Winner for Urgent Care/Walk-In Clinic in the 2026 Des Moines’ People’s Choice Awards, marking the organization’s fourth consecutive year of recognition by the community.
Since first being recognized in 2022, Hy-Vee Health Exemplar Care has earned two Gold awards and two Silver awards, reflecting continued trust from members seeking accessible, relationship-driven healthcare.
Unlike traditional urgent care clinics, Hy-Vee Health Exemplar Care combines same-day and next-day care with a membership-based direct primary care model that gives members ongoing access to their provider, transparent pricing, and care that extends beyond a single visit.

Founded in West Des Moines in 2019, Hy-Vee Health Exemplar Care has grown to 15 clinics across five Midwest states, providing direct primary care through flexible membership options that include preventive care, chronic disease management, urgent care, annual physicals, routine labs, and 24/7 access to a provider.
While the organization is recognized for delivering convenient urgent care services, its model is designed to help members build long-term relationships with their care team, reducing barriers to care and supporting better health over time.
