WEST DES MOINES, Iowa (May 15, 2026) — Hy-Vee Health Exemplar Care announced today the opening of a new clinic in Ames, Iowa, further expanding its membership-based healthcare model. The Ames clinic, located at 632 Lincoln Way, marks the organization’s 14th clinic overall and its eighth in Iowa, reinforcing continued growth and investment in delivering accessible, relationship-driven care across the Midwest.
Founded in West Des Moines in 2019, Hy-Vee Health Exemplar Care operates using a membership-based model known as direct primary care, which bundles healthcare services into a predictable monthly fee. Memberships can be used independently or alongside a traditional medical plan, offering patients greater flexibility and control over their care.
As demand continues to grow for more convenient, transparent healthcare options, the Ames clinic brings Hy-Vee Health Exemplar Care’s model closer to more communities. It provides easier access to care without the complexity of traditional fee-for-service systems.
