Dennis’ Journey

Before Exemplar Care, Dennis went to the hospital, where he was told he was borderline diabetic. “Over time, my A1C kept going up, but my primary care provider didn’t take action,” he says. “From there, we worked together to find the right medication and dosage that wouldn’t upset my stomach and would actually bring my A1C down. Now, my A1C is at a good level, and I feel better than ever,” Dennis says.
For Dennis, managing diabetes requires regular A1C checks, consistent monitoring, and ongoing communication with his primary care provider. “There has never been a time I needed to be seen that I couldn’t speak to or see my provider the same or next day,” he says. “That kind of access has made all the difference.”
