We want to make sure that your visit is as easy as possible.
If the visit relates to a worker’s compensation claim, we can accept an appointment as long as written confirmation of the following details is provided during scheduling or at the time of check-in:
- Your employer’s name and contact information.
- Your employer’s insurance information.
- Your claim number.
- The name of your claim adjuster.
- Your claim adjuster’s phone number or contact information.
We understand that sometimes information can be missing or incomplete.
If we are unable to verify your worker’s compensation claim due to missing details, we will need another form of payment to be provided at the time of the visit.
