Internal medicine physician Jon Van Der Veer, DO, started Exemplar Care in Des Moines, Iowa, to give patients access to effective, affordable health care they deserve. Dr. Van Der Veer understands how his investment of time and attention to patient-focused care makes all the difference. Dr. Van Der Veer is certified by the American Board of Internal Medicine. Before starting his own practice, he served as chief medical officer at both Greene County Medical Center in Jefferson, Iowa, and CareMore Health in Des Moines, Iowa. Dr. Van Der Veer also worked as a primary care provider at UnityPoint Clinic Internal Medicine in Des Moines.
Earning his medical degree at Des Moines University College of Osteopathic Medicine, Dr. Van Der Veer completed his residency in internal medicine at Central Iowa Health System – Methodist in Des Moines. In addition to his clinic work, he is the co-author of several articles published in medical journals.
Elli started at Hy-Vee Health Exemplar Care as a Direct Primary Care Physician Assistant in December 2020. She graduated with her Master of Science in Physician Assistant Studies from College of Saint Mary in Omaha, NE in August 2020.
When she heard about what Dr. Van Der Veer was doing and the ways he wanted to change healthcare, she could not resist the opportunity. She believes taking care of the patient should always come first, and she felt that Hy-Vee Health Exemplar Care would be the best place to live this out. Elli has loved working with the team and getting to know all of her patients on a more personal level. She is passionate about delivering high-quality and personalized healthcare to all of her patients, and is looking forward to growing with Hy-Vee Health Exemplar Care.

When she is not at work, Elli loves spending time with her husband, children and her dogs, working on home projects, and traveling.
Leah began her healthcare journey as a CNA in 2012 and completed her undergrad at Purdue Global University in 2019.
Over the next 6 years, Leah gained expert knowledge and experience as a bedside med-surg nurse at UnityPoint hospitals. She went on to finish her Master's degree and became a Nurse Practitioner with a family medicine focus in 2023.
Leah grew up in Huxley, Iowa and graduated from Ballard High School. In her free time, Leah likes to spend time with her son, go to concerts, and enjoy the outdoors.
Brach brings a wealth of experience and compassion to her role at Hy-Vee Health Exemplar Care. A proud graduate of Grand View University, where she earned her Bachelor of Science in Nursing in 2009, Brach continued her education at the University of Phoenix, completing her Master of Science in Nursing in 2012. Her commitment to lifelong learning led her to Clarkson College, where she earned her post-graduate Family Nurse Practitioner certificate in 2014, and later to Purdue Global, where she achieved her Doctor of Nursing Practice in 2018.
With a background spanning family medicine, urgent care, geriatrics, and healthcare leadership, Brach is dedicated to providing individualized, accessible care that removes traditional healthcare barriers. She was drawn to Hy-Vee Health Exemplar Care because of its direct primary care model, which allows her to spend more meaningful time with patients; listening, educating, and empowering them to take an active role in their health.
Outside of work, Brach enjoys spending time with family and friends, traveling, staying active, and exploring the outdoors. She approaches life with the same energy and curiosity she brings to her practice—always ready to learn, grow, and inspire those around her.
Jesse is originally from Michigan and graduated from MidMichigan Community College with her Associate’s degree in Nursing in 2003.
She spent the next 13 years learning and practicing Medical/surgical, ER, ICU, Telemetry, and Pediatric medicine in a small rural hospital. After graduating from Walden University in December 2016 earning her Master’s of Science Degree in Nursing with a Family Practice focus she and her family moved to Iowa. Initially working at the Polk County Jail Medical Clinic, she later transitioned into weight loss medicine but ultimately found her calling in primary and urgent care medicine.
Jesse strives to provide passionate and exceptional care to every patient she interacts with no matter their situation, illness, or complaint. She understands that while life can throw you curve balls accommodating your individual healthcare needs should never be one of them.
Outside of work, she enjoys spending time and getting the most out of life with her husband and two children. Together they enjoy taking on new adventures ranging from raising deer on their farm to riding quads and side-by-sides to cheering her daughter on in wrestling or her son in basketball or football.
Tiffany is a board-certified Physician Associate who has been practicing since 2017. She earned her undergraduate degree at Drake University and went on to complete her PA training at Des Moines University.

I’ve been in healthcare since 2003, becoming a family nurse practitioner in 2013 and later earning a second certification in psychiatry . I’ve always been a caregiver at heart, but my journey into nursing became personal after my daughter was born with a brachial plexus injury, leading to Erb’s palsy in her left arm. Her care involved surgeries, daily therapy, and visits with countless specialists—sometimes seeking second, third, even fourth opinions just to get answers. Through it all, I saw how difficult it was to find a provider who truly listened, cared deeply, and treated our family like more than just a chart. Siena received her undergraduate degree from Rockhurst University in Kansas City, MO (Go Chiefs!) and graduated with her Master of Physician Assistant Studies from Harding University in December of 2024. She is board certified by the NCCPA.
Siena initially planned to become a physician, but after working in the operating room at UnityPoint - St. Luke’s in Cedar Rapids, she was inspired by the role of physician assistants and chose a new career path.
She began her professional journey in biotechnology at Integrated DNA Technologies in Coralville, IA, where she pursued her passion for genomics while completing prerequisites for PA school. She also gained hands-on healthcare experience as a patient care technician in a dermatology clinic and as a pharmacy technician—experience that gave her an advantage in pharmacology classes during PA school.
Outside of work, Siena enjoys spending time with her extended family in the Iowa City area and her hometown of Springfield, MO. She has been married to her University of Iowa graduate husband since 2019, and has two golden retrievers, Rowdy and Teddy. Together they enjoy hiking, boating, and attending University of Iowa sporting events.
Haley is a board-certified family nurse practitioner with a Doctor of Nursing Practice from Creighton University. She brings a strong background in urgent care, emergency medicine, mental health, and community health—and is committed to delivering compassionate, patient-centered care.
Originally from Iowa, Haley earned her Bachelor of Science in Nursing from Allen College in Waterloo in 2018. She spent five years working as an emergency room nurse while pursuing her doctorate. In 2021, she completed her Doctor of Nursing Practice (DNP) at Creighton University in Omaha.
Since becoming a nurse practitioner, Haley has continued to serve in urgent care while drawing on her deep experience in acute care settings. She believes in delivering high-quality, conventional care while incorporating a holistic, whole-person approach that supports each patient’s unique journey.

Cheryl is a seasoned Certified Physician Assistant with over 30 years of diverse clinical and leadership experience. She has a strong background in occupational medicine, emergency care, cardiology, and family practice. Cheryl is dedicated to listening to and educating patients and is a strong patient advocate.
Cheryl spent 8 years in the Army National Guard as a physician assistant, including a year on active duty at Womack Army Medical Center. Her extensive experience and dedication to her field make her an excellent candidate for Hy-Vee Exemplar Care, where she can continue to provide exceptional patient care and contribute to the overall health and wellness of the community.
Cheryl graduated from the University of Illinois, Champaign-Urbana with a bachelor's degree in biology, followed by a master's degree of Physician Assistant studies from the University of Iowa College. Dr. Sarah Wiederholt is a dedicated family medicine physician with over 17 years of experience caring for patients of all ages. Originally from rural northwest Missouri, she’s passionate about building strong relationships with her patients and helping them improve their health through personalized, whole-person care. Born and raised in Maryville, Missouri, Dr. Sarah felt called to medicine early on. She began volunteering in local hospitals in high school and was accepted into an early medical school program as a junior. After earning her biochemistry degree and medical training at the University of Missouri, she completed her residency and joined Heartland Health (now Mosaic) in St. Joseph, where she has practiced ever since.
She enjoys caring for individuals and families, with a special focus on children and patients managing chronic conditions. She is currently pursuing a degree in functional medicine to further support her patients' health goals through proactive, integrative care.

Jason Gillespie is a ANCC board certified Family Nurse Practitioner and has practiced as a family nurse practitioner since 2011. Prior to this, he was a peri-operative nurse for 15 years after obtaining his Bachelors of Science in Nursing from the University of Nebraska Medical Center College of Nursing in 1995. He obtained his Masters of Science in Nursing/Family Practice from Ball State University in 2010.
Jason has interest in primary care/holistic care for both acute and chronic health issues (high blood pressure, high cholesterol, diabetes, thyroid) looking at the whole person physically and mentally as well as a special interest in sexual health and education, STI testing/prevention/treatment, as well as HIV prevention/testing as part of his overall health wellness approach with patients. The vision of Exemplar Care and the Direct Primary Care model of care aligns highly with my approach to healthcare to take care of the entire person/family that focuses on the person and changing primary care from a “get you in, get you out” approach and changing primary care from simply being a referral service and reinforcing and making primary care the biggest component in a persons health journey.

Her healthcare journey began in 2000 as an EMT-Basic, later becoming a Paramedic. After the 9/11 attacks, Angie felt a strong call to serve her country and joined the United States Air Force. In 2008, she returned to healthcare as a Registered Nurse, dedicating 16 years to emergency medicine—a field where she found both purpose and strength.
As an APRN, Angie has practiced in both aesthetics and primary care, but her heart has always remained in acute care. She chose to join Hy-Vee Health Exemplar Care because it allows her to provide high-quality, consistent care within a model she deeply believes in: Direct Primary Care. This role offers the ideal balance—enabling her to deliver timely, attentive care in an acute setting while building meaningful, long-term relationships with patients.
When she’s not caring for others, Angie is off exploring new places. A passionate traveler, she’s on a mission to visit every U.S. National Park.
Jeff brings a deep and diverse background in healthcare to his role as a family nurse practitioner at Hy-Vee Health Exemplar Care. His experience spans primary care, urgent care, correctional nursing, emergency and trauma care, as well as inpatient psychiatric and emergency mental health triage. This wide-ranging clinical foundation allows him to meet patients where they are—with empathy, expertise, and a commitment to whole-person care.
Jeff’s journey into healthcare began early, with more than 100 hours of volunteer service as a high school student in the Burn and Wound Care Center at St. Elizabeth's Regional Medical Center. That early exposure ignited a lifelong passion for caring for others and set him on a path of both academic and hands-on learning.
While living in Colorado Springs, CO, Jeff started his Master of Science in Nursing from the University of Colorado – Colorado Springs which he completed after moving back home to Lincoln. Prior to earning his MSN, Jeff completed his BSN from the University of Nebraska Medical Center – Western Division. His healthcare experience spans over 25 years, including work as a CNA, RN, and now FNP. Jeff also holds a BA in History and an Associate’s in Science, reflecting his well-rounded approach to both medicine and life.

He likes to joke that Wyatt is his favorite souvenir from Colorado, while Thomas is his favorite Nebraska creation. Tammy Rebhuhn is Board Certified by the American Academy of Nurse Practitioners and has been working as a nurse practitioner since 2006. Her diverse background includes years of experience in urgent care, family practice, management, and internal medicine.

K McCord has worked in trauma and emergency nursing for 12 years. He received his Doctor of Nursing Practice (DNP) from Creighton University and has worked in emergency departments as a nurse practitioner in Nebraska and throughout western Iowa. He is certified as a Family Nurse Practitioner through the American Academy of Nurse Practitioners (AANP).

Stephanie brings over two decades of diverse clinical and leadership experience to her role as an urgent care provider at Hy-Vee Health Exemplar Care in West Des Moines. With a Doctor of Nursing Practice from Creighton University and board certification as a Family Nurse Practitioner, Stephanie combines her clinical expertise with a passion for delivering accessible, person-centered care to patients of all ages.

Ashlee Morman is an Advanced Registered Nurse Practitioner with a strong background in family medicine and urgent care. She brings over five years of clinical experience, having served patients across primary care and emergency settings. Jennifer Mockus is an experienced nurse practitioner with a strong background in critical care, cardiology, and geriatrics. She is passionate about patient-centered care and helping people focus on what truly matters to their health.
Jennifer received her education from Drake University and later continued her studies at Kathleen University Online. Jacquline Thompson is passionate about providing evidence-based care to a diverse patient population. She holds a BSN and Master’s in Nursing from Chamberlain University and maintains certifications in BLS, ACLS, PALS, TNCC, NRP, and ENPC. Known for her compassion, problem-solving skills, and calm demeanor under pressure, Jacquline is committed to delivering high-quality healthcare that makes a real difference in patients’ lives.
Jacquline has experience in emergency and rural hospital settings, as well as specialized care in autism and ADHD management. She excels at diagnosing acute and chronic conditions, creating personalized treatment plans, and collaborating with interdisciplinary teams to ensure comprehensive, patient-centered care.
Abbigail Doud enjoys practicing within Exemplar Care’s direct primary care model, providing efficient urgent care services to member patients. She earned her Master of Medical Science in Physician Assistant Studies from Saint Louis University and holds national certification from the National Commission on Certification of Physician Assistants, as well as BLS and ACLS certifications.
Abbigail has experience in post-acute and primary care, managing chronic and acute conditions, performing procedures such as wound care and suturing, and interpreting diagnostic tests. She is dedicated to educating and empowering patients to make informed health decisions, delivering care that is both thorough and compassionate.
